1.  FIRST TIME REGISTRANTS / BRAND NEW TO HOCKEY (9 years of age and older)

If you have a child born between 2002 and 2010 and is registering to play hockey for the first time (9 years of age or older), you will need to complete a NEW PLAYER registration form. 

This form will go to our Administrator who will help you create your own Hockey Account.  Once the account has been created, you will then be able to enter into the online Registration system.  


If your child was registered with a different minor hockey association last year (not including the Markham Waxers House League), then an 'Electronic Transfer' will need to be complete prior to you being able to register with the UMHA.  Please click here to complete the transfer.  Once the transfer has been completed you will be notified.  Electronic Player TRANSFER FORM 


If your child is brand new to hockey, never having played in another association, the following items MUST be completed prior to the Sept. 15th in order to play. 

A.  Proof of Age
A copy of your child's birth certificate is required by Hockey Canada for proof of age.  To submit a copy of your child's birth certificate to the UMHA office, please click the following link.  Birth Certificate Submission Form

B.  Parent Respect in Sport Course
The Ontario Minor Hockey Association (OMHA) requires that at least one parent or guardian of each minor hockey player (18 years and under) registered in the OMHA complete the online Respect in Sport Parent Education Program.  Please click this link to be directed to the Parent Respect in Sport Page
